Focus on preparing young people for upcoming transitions following a period of time out of formal education - this may include support to return to their current education setting or preparation to move on to alternative providers or the next phase of their education.
Year 11 pupil has been out of formal education due to health needs who needs support to develop confidence and preparedness to access post-16 provision.
Young person who has been unable to attend their named setting due to anxiety or Emotion-Based School Avoidance, who would benefit from building a relationship outside of the school environment and then build on a transfer into setting.
Through discussion with setting, family and young person, build an understanding of the young person’s presenting barriers and concerns.
Deliver a personalised, adaptive programme drawing on restorative and relational approaches to build and repair support networks following relationship breakdowns,. Using CBT-based approaches to identify personal barriers and work with the young person through phased introduction and progressive steps to build engagement.
